Abstract

The partner school lacked trained peer educators, integrated educational media, and a smoke-free policy explicitly covering conventional and electronic cigarettes to support prevention from school age. A participatory empowerment program was conducted at SD Muhammadiyah 4 Terpadu Samarinda from 27 July to 15 August 2026, involving 10 students in coordination, advocacy, interactive training, peer-education simulations, development of digital messages incorporating Islamic values, and evaluation before and after training. The program established 10 Healthy Islamic Digital Cadres (KADIS) through an assignment letter, produced a smoke-free decree covering both cigarette types, provided educational resources, and initiated peer education under teacher supervision. The partner participated in cadre selection, provision of facilities, scheduling, message review, and activity supervision to support implementation within the school. All cadres completed training and simulations and developed antismoking messages for educational activities. The proportion with good knowledge increased from 30% to 100%, positive attitudes from 90% to 100%, and questionnaire-based preventive actions from 60% to 100%. A printed module, a QR-linked electronic module, posters, an information board, and a video were made available as reusable learning resources. The program strengthened initial cadre capacity and school prevention governance, while sustained benefits require teacher supervision, regular education, and behavioral monitoring because its effects on long-term smoking behavior have not been measured.
